Caring for Socially Complex Patients using Remote Care: An FQHC Use-case

Caring for Socially Complex Patients using Remote Care: An FQHC Use-case

While regulatory changes have made telehealth easier to use during COVID-19 and social distancing mandates, little guidance or best practices have been shared with providers at the front lines of patient care. FQHCs face additional complexity as they struggle to deliver clinical services to socially complex patients who already face barriers to care access and resources. During this hour-long session, Dr. Terry Sullivan, a clinical expert in technology-enabled remote care workflows, and Dr. Brian Williams, CEO, Four Corners Primary Care Centers will share their insights into remote patient care best practices specific to FQHCs. Attendees will gain a deeper understanding of the types of technologies available, how these technologies can be leveraged beyond the episode of care to ensure patient engagement, and the potential return on investment that a robust remote patient care solution can provide. By the end of this session, attendees will be able to:

  • Define the term “remote patient care” and the core components that comprise a remote patient care workflow
  • Describe the technology and patient touchpoints within a remote clinical workflow
  • Align the use of remote patient care workflows with a return on investment—across prevention efforts and recouped revenue
  • Apply remote patient care workflows to their own facilities
